Assemblies of God Water Baptism Application

Water baptism is the public act of being immersed in water as a statement of your faith in Christ. It's a special celebration in the life of a Christ follower. Congratulations on taking the next step in your faith! Please fill out the form below completely and let us know any questions you have so that we can address them.

Baptism takes place every few months, and by filling out this form, you will be added to the list of those who would like to be baptized. We will contact you as we approach the date with more information.

If there is time, we will try to produce a t-shirt in your preferred size for you to wear during baptism! Please bring a dark colored t-shirt to wear for the baptism as a backup.

There will be a mandatory orientation prior to the baptism. Please confirm your attendance below. *
